Getting Started

We start by creating a plan based on your specific needs.

The Hallmark of HomeTeam is our CareTeam. You, your loved ones, your doctor and HomeTeam work together to create a plan specific to your needs. Your plan, once established, is executed by CareTeam Coordinator. A team is selected, approved by you, and trained for your specific plan prior to any caregivers entering your home.

The plan can include things like medication reminders, personal care, light housekeeping and laundry schedules all arranged based on your needs. Nutritional concerns and special diets with meals preparations are also incorporated. Transportation and shopping trips can be also established. Daily tasks like bill pay and correspondence are also available. Finally as part of the care plan, we develop appropriate emergency responses based on the level of intervention desired.

Services for the Asian community with bi-lingual caregivers under the direct supervision of our Asian CareTeam Coordinator is the latest avenue we have opened to touch others within the community.

Taking The Next Step: A Phone Call

The initial phone call is very important. Through a series of questions including age, living situation, current health concerns, home location etc. we get a basic picture of the services that may be needed. We then schedule an onsite RN visit to complete a health and safety assessment looking for things that can be done immediately to assist in keeping you at home.

If the decision is made that we are going to provide care, we start the process of matching a CareTeam care provider with the client. The client is given the choice as to whether they want to interview the suggested CareTeam care provider. Depending on the client’s needs, we can provide just-in-time supplemental training to the care provider on the specific diagnosis (ie: Alzheimer’s care, feeding tubes, Hoyer lifts etc.)
